Ask for a seat
Leave your name, the email your Apple Account uses, and a number. Seats go out in small batches, so this is a request rather than a guaranteed seat.

The invite arrives
If there is room, TestFlight emails the Apple Account address you gave. That is why the form asks for that one and not another: an invite to any other address never shows up.

Use it on your own shopping
Accept in the TestFlight app, plan a real week, and tell us what was wrong. New builds land with a note on what changed, and every report is read by the people who build it.

Your details, and one acknowledgement.
It will have bugs. Plans and prices are estimates, and features can change or disappear between builds. That is what the checkbox says, and it is true.
